TERMS & CONDITIONS

I'm going to complete the intake form to the best of my ability and knowledge and agree to inform the technician of any changes in the information. I have been informed of and understand the contraindications to the requested treatments and agree that I do not have any condition(s) that would make the requested treatment unsuitable. I will inform the technician of any discomfort I may experience at any time during my treatment to allow them to adjust accordingly. I agree to waive all liabilities toward my technician and the employer for any injury or damages incurred due to any misrepresentation of my health history.

CANCELLATIONS

You must cancel or reschedule your appointment 48 hours prior to your appointment date. Failure to do so will result in a $25 charge. No shows are charged 50% of the booked service. If you are more than 10 minutes late, you will be charged the no-show fee (50%) and asked to reschedule.

PAYMENT

Payments can be made online or in person. The balance stated is to be finalized the day of your appointment. Payment types include: Cash or Card, Paypal, Zelle, CashApp, Venmo.

If another payment method is preferred please contact me to give direct deposit information.

PHOTOGRAPHS

Kate Aglaia reserves the right to take photographic or film records of any service performed, and may use any such photographs for promotional and/or commercial purposes.
